Authorized retail business operators can have an account with Medusa. However, to purchase certain product categories, you may be required to provide additional licenses and/or documents.
In addition to a Federal Tax ID Number (also known as an Employer ID Number - EIN), ONE of the following documents are required to have a wholesale account with us:
Sales and Use Tax Registration Certificate
Form ST-4 Sales Tax Resale Certificate
The following additional documents are required to purchase certain vapor products from us:
Retailer License for Sale of Electronic Nicotine Delivery System
Distributor License for Sale of Electronic Nicotine Delivery System
