Oklahoma Wholesale Requirements

Authorized retail business operators can have an account with Medusa. However, to purchase certain product categories, you may be required to provide additional licenses and/or documents.


In addition to a Federal Tax ID Number (also known as an Employer ID Number - EIN), ONE of the following documents are required to have a wholesale account with us:

Sales Tax Permit

Uniform Sales & Use Tax Resale Certificate


The following additional documents are required to purchase certain vapor products from us:


Sales Tax Permit with E-Cigarettes/Vapor Products Endorsement

Note: In the State of Oklahoma, wholesalers are not required to submit additional documents to purchase certain vapor products from us.
